Patient safety, at its simplest, is the attempt to understand, reduce and ameliorate the harm to patients caused by healthcare rather than disease. There is an increasing number of books, reports, articles and websites devoted to patient safety but the first edition of this book was the first straightforward overview of the field. The book describes the landscape of patient safety: how it evolved, the research that underpins the area, the key conceptual issues that have to be addressed, and the practical action needed to reduce error and harm and, when it does occur, to help those involved. Changes to the new edition The second edition will have an even stronger practical emphasis on what canbe done to improve the safety of healthcare. The first edition contained fourchapters on these themes, whereas the second will have seven, about 50% of the book. The major new themes to be introduced are a greatly expanded discussion of team based initiatives and the experiences of transforming whole organisations and health economies to bring safer care to patients.
