People with Parkinson's disease (PD) suffer from progressive impairment in their mobility. Balance dysfunction underlies gait-related disability and falls, the most important cause of physical and psychosocial disability. The recognition and evaluation of balance dysfunction by the clinician are an essential component of managing PD. Too often, balance is an overlooked component of gait disturbances in PD and turning is almost never measured in clinical settings. Few non-specialists appreciate the complexity of neural control of balance, based on the last 30 years of research. Patients with Parkinson's disease are a good model for understanding what can go wrong with neural control of balance because every balance system is affected.
